Is the above link the document you have to fill out at the medical test when re-applying for your license, or is there another longer one?
I thought I’d seen somewhere that it included questions about whether a Dr had ever advised you to cut down, and whether you’d ever drank over 8 units in one session.... but it’s not on this.
I think the above link is the same as the on on the HRO page on this site.
