How to make a complaint about the DVLA medical assessment process

Hope someone can shed some light on this,on your replication for your driving licence and you answered yes to any previous alcohol issues,do dvla send send you out a questionnaire before the medical or is it par of the medical,ie. do they write to do their necessary checks first?
