Was banned for 2 years in August 2012 on a provisional licence

I'm sat with my D1 form and I can't decide what applies to me and what doesn't. I can't send off for my first provisional licence as it wouldn't be my first. So am I to 'renew' my licence after disqualification?

I'm classed as an HRO.

There doesn't seem to be any option to renew my provisional licence, am I looking at this form too hard?
