DVLA Cover Note! Found this on Google....

Hello everyone, waiting for my medical appointment and thought id research a little on the Cover note that dvla give you if you wish to drive before the med results (if you were banned before june 2013)

Is this for anyone who has had to use a cover note what it actually looks like?

I believe that the wording is similar in a letter of authority that DVLA send out now, but the wording on the one you have provided the link to is for goods vehicle applications. You would get a letter that includes authority to drive the class of vehicles that were on YOUR licence prior to your ban.
