Help after court

My husband has recently been disqualified from driving for failing to provide. My question is where do I send his driving license to(Dvla dept)? Also what do I write as a covering letter to them? It is nearly 2 weeks since his court appearance and we need to return this within 28 days. Any help would be appreciated.

Send it with a short covering letter to DVLA, Swansea SA99 1TU. Just say: "I was disqualied at xxxxx court on xx xx xxxx date and I surrender my driving licence, please find it enclosed."
That is the section that deals with High Risk Offenders.
