Does anyone know what to do if you lose your certificate of completion?

Insurance company have refused to renew so now have to go elsewhere and they want to see a copy but it's missing

I would advise you to contact the course provider who you completed the course with. A list of drink driving rehabilitation course providers along with contact details and the geographical areas they cover can be found here.
